The Coordinated Care Platform

We provide a best-of-breed technology platform which enables administrative and clinical integration across networks of health care providers and their health plan partners. This makes it easier and more efficient for community-based care teams to communicate and collaborate on health, wellness and treatment plans. Patients, providers and health plans are better able to manage transitions between care delivery sites and predict and manage appropriate care interventions along the way.

The Health Access Solutions Coordinated Care Platform™ harnesses the integrated capabilities of Access Express™, a widely deployed care management solution, and Excelicare™, an industry-leading clinical management and advanced Type 5 HIE solution, which was recognized by Gartner as one of its most exciting technologies for 2011. These products are also available as independently operating solutions.

The Access Express™ Care Management solution has been developed with more than a decade of direct experience, serving provider networks operating in a delegated risk-sharing model similar to that of emerging ACO-style organizations. The platform also enables robust quality performance reporting and population health management with real-time analytics and predictive risk management support.

The Excelicare™ Clinical Management solution has been in production across the UK for more than a decade, supporting the National Health Service’s (NHS) care-team management of chronic patients across the care continuum. In 2005, Excelicare was chosen as the National Generic Clinical System for the NHS in Scotland. In 2010 Excelicare was selected as the key software solution provider in a major eHealthcare project for managing chronic diseases for the population in the province of Ontario, Canada.

  • Intuitive, multimodal communication portals for patients and the care team. Modalities include Web, secure email, text, IVR and others
  • Context-driven delivery of medical and administrative data to support clinical decision making and care management
  • Evidence-based decision support and care plans at the point of care
  • Judicious use of real-time quality performance dashboards, treatment reminders, care gap alerts, medication and care plan adherence notifications
  • Bi-directional secure messaging among the care team and patients
  • Patient access to summaries of their most current clinical information
  • Patient entry or interface of their own clinical and behavioral information, such as blood glucose readings, daily weight and other self-managed wellness data
  • Medical management protocol rules engine making it easier for patients to stay “in-network” — by appropriately managing provider referral and treatment authorizations (Shared Decision Making) based on objective criteria such as satisfaction scores, quality and cost performance
  • Workflow and communication tools for onsite and complex case managers to assist the patient and care team with appropriate utilization of services and coordinated care transitions.
