Founded in 2000, California-based Health Access Solutions enables coordinated care by delivering solutions that assist the multidisciplinary care team in delivering accessible, high-quality care while carefully managing cost.
At Health Access Solutions, we understand the challenges facing the industry today—and recognize that patient-centric models such as Accountable Care Organizations and Patient Center Medical Homes are key to transforming and reforming healthcare. Relying upon a genuinely patient-centric model, our Web-based platform is designed to allow physicians, hospitals, and health benefit and payer organizations to connect, collaborate and tightly coordinate healthcare delivery—to promote efficient care management, enhance quality of care and improve clinical outcomes. Today we serve 27,000 physicians along with their care teams and staff, which collectively treat and manage more than 3.5 million patients and health plan members.
